Why are we asking you to complete this survey?

We have asked you to complete this survey as you receive Day Opportunities from your local authority Social Services department. In order to ensure that the service provided meets your needs we ask that you complete this survey to evidence your journey. We will use this information to improve services to ensure that they meet your needs.

What would we like you to do?

Please complete the short questionnaire. The questions ask you about how the services you have received have helped you.

How will we use your answers?

Your answers will not affect any care and support you receive. We will only use the reference number on the questionnaire to let us know that you have replied so we do not ask you to complete it again. We will provide anonymised answers to the Welsh Government so they can monitor how well we are providing our services.
